Within Public Health Nursing, collaboration amongst an interdisciplinary team is essential in providing holistic care for the populations served (Dahl & Crawford, 2017). There are a variety of methods of collaboration between internal and external stakeholders that I have observed in my practicum setting. One method is through the use of interdisciplinary teams. These teams are composed of representatives from various disciplines who work together to address the client’s needs. Another method is consultation, when representatives from different disciplines meet to discuss the client’s needs and develop a care plan. Finally, I have also observed collaboration through case management. Case management is when a team supports the client to help them meet their goals and objectives. All of these collaboration methods are important to provide quality care for the client as they help to ensure that the client’s needs are met and that they receive the best possible outcome.
The benefit of their method of collaboration is that they can provide a comprehensive plan of care for the client, which is designed to meet the client’s individual needs and is tailored to their specific situation (Dahl & Crawford, 2017). Additionally, these collaboration methods allow for better communication between different stakeholders, which helps ensure everyone involved in the client’s care understands their needs. However, the challenge with their method of collaboration is that sometimes it can be difficult to get all parties involved in the client’s care on board with a particular plan of action, as well as difficulty tracking progress and outcomes for the client as a result of these collaborations. Much of the communication occurs through an internal messaging system for non-urgent requests. This can sometimes delay collaborative care and the care provided to the patient.
